They are just illegal hold items that add damage to their attacks. Nothing more. There's not going to be blood or Pokemon visibly holding giant swords or anything. It'll be more like, "Electabuzz inflicted extra damage with DAGGER!" or something to that effect.

That's correct. I think one starter isn't really enough. Pokemon is about team management, and starting you off with more than one Pokemon will add more strategy to earlier battles. One would also be slow, the other fast. It's a good way to jump into the strategy.

If it adds strategy, why are they disabled in multiplayer? It detracts from strategy, it allows you to spam items and basically undo an opponent's hard work on you. It's disabled in multiplayer to make the battle both fair and challenging, and that is why I decided to disable it in single player as well.
Pokemon Centers would not be as plentiful in this game as they were in previous ones, and it's annoying to run back to a Pokemon Center every time your Pokemon get hurt. It doesn't make sense that your Pokemon can run to you during a fight, and you can spray medicine on it to heal it, and that's OK. That being said, Black Riot would be using items during battles because they cheat.
Either way, I think removing items from battle is a good idea.
