what did Leo mean when he said "living as a monster or dying as a good man" at the end?

Was he going to pretend like he hadn't made any progress so the hospital or whatever would kill him, so he'd die as a "good man" (in his own mind) instead of living as a "monster"?

This movie is amazing! It has changed my view on people with extreme mental health problems. It really puts you in his place so you're on his side until the end .. where it hella turns! And, at the spoiler: yes, I'm pretty sure he does do that (and for that reason). Well, I got that impression from it anyway ...
